A.  The Grantee shall provide a detailed budget breakdown clearly distinguishing between cash 
    and non-cash (in-kind) contributions, with the submittal of the first payment request.
B.  With each payment request, submit a detailed schedule reflecting your cash and in-kind
    expenditures (mast be identified separasety) fur not only the current period but
    cumulatively as welL Tlsis schedule must also include a comparison uf your approved
    match budget (by line item detail) to actual cumulative match expenditures.
C.  Ante(s) must he dedicated In this contract or agreement and will consist of the folowing:

    1. For any declared in-kind contribatiom, the following documentation will be maintained
       in the file(s):
       (a) An itemized listing of each employee’s hourly rate. (Prevailing Wage Rates may be
            used if applicable)
       (b) A Fringe Benefit detail and explanation.
       (c) A copy of an appmved Overhead/Indirect Cost Allocation Plan.
       (d) An itemization of’per diem rates, equipment rental/usage rates, etc. that will be 
           used us match fur this contract or agreement
       (e) Copies (or origltsals) of time sheeta, with employee’s and supervisor’s signature,
           noting dates, hours, and pm)ecta worked.
       (f) Copies (or originals) of logs/schedules fur equipment usage.
       (g) Signed statemente noting fair market value for in-kind dunatiom of materials or 
           supplies.
    2. For any declared cash contributions, the following documentation will be maintained in
       the file:
       (a) An itemization of each employee’s hourly rate including fringe henefits, overhead,
           and indirect cost
       (b) An itemization of per diem mtes, equipment rental/usage rates, etc.
       (c) Copies (or originals) of time sheeta, with employee’s and supervisor’s signature,
           noting dates, hoams, and pmjecte worked.
       (d) Copies (or originals) of logs/schedules for equipment mage.
       (e) Copies (or originals) of invoices for materials, supplies, equipment, etc.
D.  The Grantee agrees and acknowledges that:
    1. Neither the costs nor the values of third party in-kind cuntributiom being used to 
       satisfy the cost sharing or matching requieementa of the attached Grant have heea or 
       will he counted towards satisl/jing a cost sharing or matching requirement of another
       federal grant agreement, a federal pmcarement contract, or any other awani nf federal hinds.
